Blemishes appear when sebum, dead skin cells, and bacteria accumulate in pores, triggering localised inflammation. What is happening

Blemishes form when the sebaceous glands produce excess sebum, often triggered by hormonal shifts, stress, or barrier disruption. This sebum mixes with dead corneocytes that have not shed properly from the follicular wall. The result is a microcomedone, a microscopic plug that blocks the pore. When Cutibacterium acnes, a resident bacteria, proliferates in this anaerobic environment, the immune system responds with localised inflammation. White blood cells migrate to the site, producing the visible papule or pustule. The surrounding tissue reddens as capillaries dilate. Once the lesion resolves, melanocytes may overproduce melanin in response to the inflammation, leaving behind post-inflammatory erythema or hyperpigmentation. The cycle often repeats if the skin remains in a state of excess sebum production, impaired desquamation, or compromised barrier function. Topical irritation from over-cleansing or harsh actives can worsen the cycle by further disrupting the acid mantle and microbiome, prompting even more sebum as a compensatory response.
